56. Caesarean section on maternal request.
Candidate’s instructions.
You are a SpR5 in the antenatal clinic.
Your consultant is feeling unwell and has gone to lie down.
The midwife has just seen a primigravid woman who has requested Caesarean section.
She is healthy, with no significant medical history and the pregnancy has been normal.
The gestation is 36 weeks, the head is engaged and the baby seems to be of an average size.
The midwife has done all the routine investigations and has asked you to see her to discuss the request for Caesarean section.
It is your task to discuss her request as you would in a normal clinic.

57. Incomplete audit.
Candidate's Instructions.
Incomplete audit.
You have been given the work done on an incomplete audit and have been asked to complete it.
The data you are given about the audit are:

Consultant A offers ECV from 35 weeks & has a success rate of 20%.
Consultant B offers ECV from 36 weeks in primigravidae & 37 weeks in mulitparae & has a success rate of 50%.
Consultant C does not offer ECV.
Discuss with the examiner what conclusions you can reach so far and what additional information you need to complete it.

58. Cannabis use in pregnancy.
Candidate’s instructions.
You are a 5th. year SpR and in the booking clinic. The midwife has asked you to see Alice Spring who has booked in at 10 weeks in her first pregnancy. She is well and the main concern is that she has reported smoking cannabis most days. The midwife does not feel that she knows enough about cannabis use to advise her.
Your task is to deal with Alice as you would in a real antenatal clinic.

59. Critique RCOG’s Pt. Info leaflet on Genital Herpes.
Candidate’s instructions.
This is an unstructured viva. Read the leaflet and make a critique. The examiner will not ask questions or help in any way.

60. Headache.
Candidate’s instructions.
This is a structured viva station about headache. The examiner will ask you 13 questions. You are not allowed to return to earlier questions as later questions may provide the answers. If you do return, no marks will be allocated, even for correct answers. The examiner will not give you any assistance – it is your responsibility to manage the time available well.
61. Surrogacy.
Candidate’s instructions.
This is a structured viva station about surrogacy. The examiner will ask you 16 questions. You are not allowed to return to earlier questions as later questions may provide the answers. If you do return, no marks will be allocated, even for correct answers. The examiner will not give you any assistance – it is your responsibility to manage the time available well.
